I/We would like to purchase a(some) Freewheeling jersey(s) and have it(them) waiting for me on my tour. The jerseys are available for C$95, S-XL in "PRO FIT" and "RELAXED FIT".

Cancellation Insurance:

	I/We would like to purchase cancellation/travel accident/baggage & personal effects insurance (from RBC Insurance of Canada; approximate cost = 6%).
	I/We do not want cancellation insurance. I/We understand that we may lose deposits if we leave or cancel our trip. We have traveller's accident coverage through another source.

Waiver:
We acknowledge that our trip or excursion with Freewheeling Adventures will be subject to the terms and conditions of Freewheeling Adventures standard form of Agreement. We understand that these terms include that Freewheeling Adventures will not be held responsible for any loss or damage suffered by any person, in travelling to or from the trip, or before, during, or after the trip, for any reason whatsoever. We also understand that all risks involved in the taking of the trip are assumed by the participant, as are costs incurred for emergency services. We acknowledge that the form of the Agreement is immediately available at the offices of Freewheeling Adventures, and will be provided to us as part of our booking materials, or immediately for our review upon request. We understand that we must sign a copy of the Agreement with its complete terms and conditions before commencing our tour.
